Enrollment

If your child will be new to Horizon Elementary, please print, complete, and return the appropriate enrollment forms to the school as soon as possible. 

In addition to these forms, we will also need a copy of your child's birth certificate and a copy of your proof of residency (e.g. utility bill or front page of build/sales contract). Your child's doctor can provide a copy of immunization records in place of the Kansas Certificate of Immunizations and a copy of your child's latest physical in place of the Health Assessment for Children and Youth form.
Physicals are only needed for children who are under the age of 10 and moving from out of state.

Once we receive all of your registration documents, we will enter your student(s) into our system and send you your Skyward Family Access login information. Skyward is where you can pay district/school fees, add money to lunch accounts, and view your student's grades, attendance, and more! You are welcome to scan and email your enrollment documents or drop them off in the front office.
